- Letter to GP:
Immediate Discharge Letter outlining admission details, investigations, interventions and discharge medications.
Formal discharge letter will follow with more detailed outline and follow up for patient.
Also require follow up arrangements with cardiology, medication instructions and detail if she has received input from rehab team and when GP should follow up and with what e.g. bloods etc.
The SIGN (128) discharge document has now been published which gives clear guidelines to adhere to for discharge letters in Scotland. This can reviewed at www.SIGN.ac.uk - Cardiac Rehabilitation:
Many hospitals have a cardiac rehabilitation programme, run by cardiac rehabilitation nurses and physiotherapists, which will help the person regain fitness and confidence as well as providing information and advice.
This type of programme can be adapted for the person whatever the level of fitness.
On leaving hospital Joan should be provided with information and direction to evidenced based cardiac advice. Joan’s psychological health will be considered as part of her holistic ongoing management.
We will discuss this more within module 5. - Cardiology Clinic:
Follow up at Cardiology Clinic as per local policy. Within most areas this follow up clinic will be accessed within 6 weeks and may consist of;- Observations (manual pulse, blood pressure)
- Electrocardiogram (ECG)
- Physical examination
- Echocardiogram (ECHO)
- Exercise Tolerance Test (ETT)
Following this assessment a decision will be made regarding her long term management.
