- Letter to GP:
Immediate Discharge Letter outlining admission details, investigations, interventions and discharge medications.
Formal discharge letter will follow with more detailed outline and follow up for patient.
Also require follow up arrangements with cardiology, medication instructions and detail if he has received input from rehab team and when GP should follow up and with what e.g. bloods etc.
The SIGN (128) discharge document has now been published which gives clear guidelines to adhere to for discharge letters in Scotland. This can reviewed at the Scottish Intercollegiate Guidelines Network (SIGN) website. - Cardiac Rehabilitation:
Many hospitals have a cardiac rehabilitation programme, run by cardiac rehabilitation nurses and physiotherapists, which will help Naveed regain fitness and confidence as well as providing information and advice. This type of programme can be adapted for the person whatever the level of fitness.
Naveed’s psychological health will be considered as part of his holistic ongoing management as he had complications in terms of his cardiac arrest. Naveed will be supported to consider ongoing risk factors that he could begin to address including healthy lifestyle changes, physical activity and alcohol intake.
We will discuss this more within module 5. - Cardiology Clinic:
Follow up at Cardiology Clinic as per local policy. Within most areas this follow up clinic will be accessed within 6 weeks and may consist of;- Observations (Manual pulse and blood pressure recording)
- Electrocardiogram (ECG)
- Physical examination
- Echocardiogram (ECHO)
- Exercise Tolerance Test (ETT)
Following this assessment a decision will be made regarding his long term management.
