The volunteer driver brings Jean to the tea-dance. Cardiac rehabilitation input has given Jean confidence and knowledge that having a cardiac event does not mean she has to stop enjoying social activities.

Jean chats to Maggie at the tea dance

Maggie ” Awe Jean it’s good to see you, I wasn”t sure if you would have been fit to make it? ”

Jean ” Neither did I, but I am feeling much better now. Paul the local volunteer driver picked me up from the house and dropped me off”

* reference made to SW referral made by CR nurse to local volunteer driver service – assistance to combat isolation, mobility issues.

Maggie “That’s good, did the nurses tell you to take it easy?”

Jean “No not at all, I was referred to a physiotherapist so I could do some exercise at home and then build up gradually. I made a plan with the nurse and it was all done at a pace I could manage” *

*reference to referral to community physiotherapist; goal setting with CR team adopting a “client centred approach” to insure the goal setting is SMART

Maggie “How are you managing at home? Are you remembering to take all those new pills?

Jean “A lot better than I was, and I’m eating better too; I got a new hand rail put in to help me with the stairs… My pills come in a “blister pack” to help me remember what to take and when to take it” *

* reference to referral to community OT and support provided via additional aids. Community pharmacist can provide a dosette box or blister pack to help with medication adherence.

In meeting these defined goals, all cardiac rehabilitation programmes should aim to offer a service that takes a multidisciplinary bio-psychosocial approach in order to best influence uptake, adherence and long-term healthier living for each individual patient the involvement of a partner, other family member, carer, close friend (i.e. a significant supporting other) is also important.
