What should the nurse do now?

Q. After this initial assessment what should the nurse do now?

  • Assist Molly out of the surgery and advise her to phone her GP when she gets home
    Wrong – Molly is too unwell for the nurse to make this decision and requires further assessment today
  • Refer to GP
    Wrong – this could take some time and she needs a review and management plan today
  • Follow up appointment with nurse in 1 week
    Wrong – a week is a long time for someone with heart failure and worsening symptoms there hasn’t been a nurse assessment done yet and she needs reviewed today
  • Call an ambulance
    Wrong – at this stage we don”t know if Molly needs an ambulance as she has not been fully assessed
  • Ask a GP to come in and review urgently
    Correct – Molly requires further investigation as to why she is feeling worse and merits a timely review.
