Important environment/functional factors in John’s assessment. Select the crosses for more information.
John’s functional ability – This may be contributing to johns ability to manage continence. Things to consider are:
- can they dress self
- can they pull down garments and redress self
- can they wipe and clean self following toilet
John requires assistance with garments, doing up buttons and unable to pull garments down. Has residual right upper limb weakness. Initially dysphasic but able to now tell staff what he wants.
John’s mobility – This may limit Johns ability to access/mobilise to toilet. Consider:
- use of aids or any assistance required
- Johns fear of falling
John walks to the toilet using a walking frame. He requires assistance with standing up from the toilet.
Environment factors – These may contribute to Johns ability to manage continence consider:
- ability to use call buzzer
- location of bed in relation to toilet
- access to toilet
- positioning of equipment in toilet
John’s bed is positioned furthest from the toilet. He has difficulty in locating the call buzzer as it is often left on the opposite side, as is the urine bottle.
