Four main strategies have been devised from the feedback at the MDT meeting regarding Isa’s medication management.
- Information – Information has been adapted for Isa, by Speech & Language Therapist, to make it more easily understood. Example of adapting information [.pdf, 376 KB]
- Opening containers – The ward pharmacist has contacted the community pharmacist to discuss their willingness to transfer tablets from foil into an easier to open container. However, this has raised issues about the stability of the tablets when removed from the foil. The ward pharmacist is checking with the manufacturers for advice about each medication being removed from their foil. Often, safe storage in a patient’s home where children visit is a concern. However, Isa has no child visitors.
- Travel to clinic – The nurse has spoken to Isa about how her warfarin will be monitored. Jean has offered to drive Isa to the warfarin clinic. In some areas INR testing is done in local pharmacies, local GP practices or by the district nurse visiting the person at home.
- Addressing Isa’s fears – The ward pharmacist has encouraged Isa to try the bendroflumethiazide. He has reassured Isa that at this dose most patients don’t experience the same problems as Effy. He has also highlighted the importance of taking it to help reduce her blood pressure.
