Examination and neurological examination:
- An irregularly irregular pulse
- HR 90 per min – BP 156/94
- Normal heart sounds
- Normal peripheral pulses and no carotid bruit
- A mild weakness of her right hand (she is right handed)
- Mild word finding difficulties and some problems with understanding
Diagnosis:
- A minor left hemisphere stroke
- Possibly related to degenerative vascular disease and/or newly diagnosed atrial fibrillation
- Patient is out with the time period for thrombolysis
Next Steps:
- Take bloods
- Arrange an ECG
- Send patient to radiology for a CT brain scan and carotid duplex
- Arrange a bed on the stroke unit
