Examination and neurological examination:

  • An irregularly irregular pulse
  • HR 90 per min – BP 156/94
  • Normal heart sounds
  • Normal peripheral pulses and no carotid bruit
  • A mild weakness of her right hand (she is right handed)
  • Mild word finding difficulties and some problems with understanding

Diagnosis:

  • A minor left hemisphere stroke
  • Possibly related to degenerative vascular disease and/or newly diagnosed atrial fibrillation
  • Patient is out with the time period for thrombolysis

Next Steps:

  • Take bloods
  • Arrange an ECG
  • Send patient to radiology for a CT brain scan and carotid duplex
  • Arrange a bed on the stroke unit
