What other tests could he have?

Clinically, Iain has had a right cerebral stroke. There are a number of tests which could now be carried out. (See the Additional Information below for further information on these tests).

Electrocardiogram (ECG) – as Iain has a pacemaker the ECG technicians could be asked to “interrogate” the pacemaker to see if he has been in atrial fibrillation. A 24hr tape can be useful to detect paroxysmal AF. However, because Iain has a pacemaker this would not be indicated in his case.

Prolonged ECG– may also be considered for some patients but because Iain has a pacemaker this would not be indicated.

Carotid duplex – since this was a right cerebral stroke he should be assessed for a possible carotid stenosis requiring carotid intervention.

Brain Imaging- CT brain scan: since he has persisting symptoms it is important to exclude a bleed as a cause of his stroke. He is currently within the time window for thrombolysis therapy, so this should be done immediately. MRI brain: he has a pacemaker which is an absolute contraindication to MRI, this would be fatal! If he did not have a pacemaker an MRI is a reasonable alternative to CT and is better at distinguishing haemorrhage from infarction more than a week after the onset of stroke. It is also more likely to show a small area of infarction than a CT so if there is doubt about whether the stroke affects the carotid or posterior circulation it can be useful.

Echocardiogram – given his history of heart disease and a murmur it is probably worthwhile.

Cervical spine X-ray – this is very unlikely to show an abnormality which would alter his management. Degenerative changes would be the norm in a man of this age.

“Young Stroke Bloods” – these include lupus anticoagulant screen, thrombophilia screen, autoimmune screen, homocysteine and syphilis serology. As Iain is older and has established vascular disease this is likely to have caused his stroke.
