Team meeting

George’s condition has not improved, he has ongoing complications such as a chest infection and the team feel his prognosis is poor. It is also clear from the discussions that there are differing opinions on George’s potential recovery in the family.

Doctor speech bubble:
I have had a discussion with George’s wife. She has told me that she is regretting the placement of the NG tube. She shared conversations she and George had in the past where she promised him she would never let him be kept alive in a such a poor state. She was very emotional and doesn’t know how her daughter will react.

Nurse speech bubble:
I am worried about George and his wife. George is getting weaker and he is not able to do anything for himself.
His daughter brought in his young grandson to see him this week and he didn’t seem to react. His daughter has been asking when he should get a gastrostomy tube as she has researched this on the internet.

Physiotherapist speech bubble:
I have been working with George for 3 weeks now and there has been very little improvement. George’s wife watched me working with him this week and she was very quiet. She commented that he would not want this.
He needed maximum support and we are actively treating his chest. He was clearly quite fit before his stroke.

Speech and Language Therapy speech bubble:
I have regularly reviewed George’s swallowing but he is making no attempt to move a bolus placed in his mouth. Secretion management is a problem. He is tolerating his NG. He has made no verbal response and has no established means of indicating ‘yes’ or ‘no’.

Dietitian speech bubble:
I have monitored George’s feeding over the 3 weeks. He has been tolerating his NG tube. He appears to have lost a small amount of weight since his admission. I believe his daughter has asked to speak to me about a gastrostomy tube. I am happy to meet with his family but thought I should get a team up-date first.
