Twelve hours later the results from Archie’s observation chart is as follows:

Q. Are Archie’s results normal? Select either normal or abnormal for each result below.

Respiratory rate (RR): 22 – Abnormal – normal Respiratory Rate (RR) is 14-20 per min at rest.

Heart rate (HR): 80 bpm, regular – Normal – normal Heart Rate (HR) is 50-80 bpm and regular.

Temperature: 37˚C – Normal – tympanic temperature is normally ≤ 37°C.

Oxygen saturation: 92% (on air) – Abnormal – Oxygen saturation is normally ≥ 95% on air.

Blood pressure (BP): 170/90 – Abnormal – Archie’s Blood Pressure (BP) has increased since his last results. However, blood pressure is often elevated (up to 220/110) after Stroke but this usually resolves without treatment.


Q. Based on Archie’s test results, what should you do now? Put 4 of the following options in order of priority.

  1. Check the saturation probe is correctly positioned on the finger – Yes – it is better for a probe to be placed on a finger rather than the earlobe, or an unpainted nail.
  2. Repeat the observations – Yes – this is correct.
  3. Inform the on-call doctor – Yes – this is correct.
  4. Increase observation frequency to hourly or more often, following  your local protocol,  – Yes – this is correct.
  • Continue 4 hourly observations – No – this is the wrong thing to do because Archie’s respiratory rate is elevated and his oxygen saturation is low, please try again.
  • Begin high flow oxygen – This may well be required but should be prescribed by a doctor, please try again.
