Charlie meets up with Dorothy

Having reviewed the unit’s procedure Charlie commences his key worker role. He explains his role and the process to Dorothy and talks through why it may help her in her rehabilitation. She has a good understanding of goal setting as she regularly set goals for herself at home and at work before retiring. She can see the value of goal setting and is keen to engage with it. He leaves her to think about what she would like to work on and gives her the opportunity to discuss it with her family.

See Additional Information box for further information on the role of the key worker.

Charlie sets aside time and meets with Dorothy to identify her main priorities in relation to her recovery. Listen to the audio clip below of their meeting.

Charlie goes on to discuss with Dorothy and her rehabilitation team each of her priorities. Together they identify her current abilities and goals in relation to each one. Charlie documents them on her goal sheet.

Full transcript

Charlie: Today we’re going to start your goal setting. What I need to know first is what are the most important things that you would like to work on?

Dorothy: Well…. You know I really just want to go home and be independent

Charlie: OK. When you say ‘to go home and be independent’ what do you mean by that?

Dorothy: Well you know to walk; to manage in the bathroom and put my clothes on; to drive again things like that.

Charlie: It’s probably a wee bit early to address the driving one but it’s a good idea to have it as a target. Thinking of your walking and just to make sure I am clear, when you say to walk what do you mean is it just inside?

Dorothy: No I want to be able to walk around my house and outside. I’m not worried if I need a stick or anything I just want to get on my feet

Charlie: That’s great. I also wondered what you meant by to ‘manage in the bathroom’?

Dorothy: Well you know go to the toilet without any help, I know I need help now but I hate having someone with me, oh and to get in and out of the bath. I do like a bath

Charlie: That’s nice and clear. It’s good to see you’ve really thought about it. Is there anything else? What about doing other things like in the …well you know cooking or going up and down stairs?

Dorothy: Mmmm yes I need to be able to go up and down the stairs for home. But cooking … no dear not for me I’m a really terrible cook my husband does that. But just as we are having this chat another thing has come to mind I would really like to get back to meeting my friends for a coffee; I did that every Wednesday

Charlie: Great. Can I just run through what I’ve got down as your current priorities and see if I’ve got it right? They are to – walk inside and outside; manage at the toilet on your own, get in and out of the bath; get dressed; go up and down stairs; meet friends for a coffee and get back to driving

Dorothy: Yes I think that’s it for just now anyway. I’m sure I will have more
