Dorothy’s goal sheet

So, what does a goal sheet look like? Let’s see how Charlie got on writing Dorothy’s goals.

Having read through the comments, Charlie takes the opportunity to meet with his clinical supervisor to discuss and reflect on how his key worker role has gone so far.

Patient Priorities: To go home and be independent:

  • To walk inside and outside
  • To manage in the toilet by myself
  • To get dressed
  • To go up and down the stairs
  • To get in and out of my bath
  • To meet my friends for a coffee
  • To get back to driving (longer-term goal)

The priorities are detailed and clear. It’s great that you have included her longer term goal so it doesn’t get forgotten.

Clear set review date which seems appropriate for these goals.

Clear simple starting point making goal identification and measurement of outcome easier.

Goals are simple; measurable and seem appropriate.

Use of patient-friendly language good. I also like the use of the patient’s name which personalises it even more.

Often with this stage goals tend to be impairment/activity related it is good that you have included a participation one as well. See ‘Additional Information’ box below.
