Charlie, Ruby and the MDT meet to review her goals. Below are Charlie’s notes, have a look at the outcome of the review.
Everybody, including Ruby, agrees that she has partially met one goal and has not managed to achieve the rest.
Ruby:
- Remembers talking to Janet about her goals but can’t remember exactly what the goals are or what she was to do
- Was unwell last week and spent a few days in bed, feeling better now but did have a few accidents at the toilet
- Doesn’t recall getting the exercise sheet the physiotherapist was supposed to give her
- Hasn’t been in the kitchen to make a cup of tea
- Has achieved organising her clothes in the morning but is needing assistance of 2 staff to get dressed
- Disappointed that she hasn’t achieved any of her other goals
MDT:
- Janet Joint (physiotherapist & key worker) has just gone on honeymoon for three weeks and was struggling to get everything done before she went off
- Mrs Graham doesn’t have an exercise sheet for the ward
- Ruth Rail (occupational therapist) has been off sick for a week
- Nobody in the team has a copy of the goals and so they aren’t sure what they are
- Mrs Graham was quite unwell with a UTI, has just finished her antibiotics and taken a bit of time to get over it
- The team feel they haven’t quite got to grips with her goals
