Telehealth is the remote exchange of data between a patient at home and a clinician(s) to assist in diagnosis and monitoring, typically used to support patients with long term conditions. Find out more about Digital Health & Care Scotland. Given his rural location and his difficulty getting time off work to attend the health centre and clinic appointments, the Heart Failure Nurse feels that Sandy will benefit from telehealth technology.
She enrols Sandy on the FLORENCE system and meets with him to show him how to use it. She sets up the system to contact Sandy 3 times a week.

The Heart Failure Nurse enrols Sandy into the Florence telehealth app
1. The HF Nurse enters Sandy’s information into the system.
2. Sandy receives text messages via his mobile phone:
Hi Sandy, just a reminder to make sure you take your tablets today. Thanks. Flo
Hi Sandy, are your palpitations more or less frequent today? Thanks. Flo
… less frequent
3. Florence reminds patients to take their own readings. The HF Nurse also sets Florence to alert her with text when readings don’t look right or show worrying trends.
Hi Sandy, are you feeling more breathless today?
Hi Sandy, have you tested your urine for glucose today?
