Research suggests that long-term behaviour change is unlikely to be sustained without the involvement of health professionals (Prochaska et al, 1992).
By taking an interest in patients’ lifestyles and communicating with them over behaviour change, all HCPs are endorsing a healthy lifestyle, enhancing patient health and wellbeing, and taking primary and secondary preventive measures. - I wonder if there is something I can change to help myself.
Supported self management can be beneficial to patients who have had health changes. Many people also say they have increased joy in being alive. They may begin to re-evaluate life goals and the value of relationships, and have an increased feeling of well being through adopting a healthier lifestyle.

Most public and private sector employers will have Occupational Health Departments with clear policies about return to work plans. People who are self employed may need to get specialist advice from their GP, depending on the type of job they do. The DVLA has certain requirements. If financial issues are a major concern, advice can be obtained from Citizens Advice or another recommended non profit agency. - What can I do to prevent having a heart attack (like my brother)?
There are modifiable factors and non modifiable factors in heart disease. By providing patients with standardised, evidence based information and facilitating behavioural change, we can clarify cardiac misconceptions and support self management.
