  • Chest pains –
    • S – the centre of chest
    • O – when climbing stairs and never at rest
    • C – tightness across chest
    • R – no
    • A – slight breathlessness.
    • T – goes away after 10-15 mins.
    • E – nothing else, better with rest
    • S – 3 or 4 / 10
  • Pulse – 68
  • BP – 130/80
  • U & Es taken– ramipril not at optimal dose
  • Forgetful? Issues with medicine adherence
  • Anxiety / depression – moderately elevated for both
  • Inactive & lost confidence – Weight checked: 48.5kg, weight loss? distracted by low mood? Continue to monitor
  • Struggling with stairs – only 1 hand rail
