Dietetic consultation

Following the group session, John has a one to one consultation with the dietitian to discuss and agree individual goals using health behaviour change techniques via the diet agenda setting tool. Acknowledging that John had highlighted this as a goal, John was encouraged to consider this more specific agenda. He shows the dietitian his completed 3 day food diary prior to this consultation.

John is setting his agenda using the food agenda setting tool. When assessing readiness for change and action the patient is engaging in dietary and lifestyle change.

John seems committed to further dietary change and for this the dietitian is giving him the tools to help him achieve his goals concerns.

Dietitian: You recently attended the group talk on healthy eating for a healthy heart. Is there anything you would like to focus on today? Have you already began to make changes to your diet? Have a look at the food for thought chart and show me what your main areas of food you eat just now are?

John: Over the last few weeks I have stopped adding sugar to tea and coffee and also now using sugar free drinks. I have changed to semi-skimmed milk and cut the fat down in my diet by grilling instead of frying and cut out the fast food and takeaways. I have now stopped smoking, this has been hard. I now find that I’m snacking more on sweets and biscuits when I have cravings for a cigarette, but my weight is coming down. I’m keen to lose more weight. I’m attending the CR exercise classes and increasing my walking and started cycling. I would like some advice and information on how to get my weight down. (Patient in Action stage of change).
