A4 sheet with meds and when to take them. Medication: Paracetemal, Wafarin. antibiotics. Warfarin book.

Discharge letter – CR referral – Dear GP Michał Lubinzki, Flat 3, 40 Morrings Road. Aortic valve.

Warfarin booklet:
Oral Health: There may be a link between oral health and cardiovascular health, more research is required but patients should be advised to tell the dentist that they have had cardiac surgery and are prescribed an anticoagulant such as warfarin.
Patients prescribed warfarin receive this booklet and it provides important information on their treatment and contact information for further advice. Anticoagulant clinic dates will add information to the booklet including latest blood results, dosage of anticoagulant and dates of future blood tests.
