HF nurse specialists manage the continuing care of heart failure patients mainly in the community setting. Their roles are primarily to educate the patient and carer into the self management aspects of their condition and to ensure that evidence based medication is prescribed but an important aspect of their role is in the early detection of deterioration and take pre-emptive action to prevent adverse events/hospital admissions.
