Your patient was admitted with a 6 week history of worsening breathlessness and chest tightness.

Principal Diagnosis/Procedure: Exacerbation of CCF, pleural effusions and chest infection.

Background: COPD IHD, Chronic kidney disease (CKD)

Treatment: Future investigations and follow up: No formal plans for follow up by hospital team. Please repeat U&E’s 7 days post discharge.

Changes to drugs since admission Previous adverse drug reactions: Penicillin – Rash

Discharge medications: Bisoprolol 5mgs Daily long term, Furosemide 120mgs twice daily long term, Digoxin 62.5mcgs daily long term, Ramipril 5 mgs daily long term, Spironolactone 25mgs daily long term, Salbutamol Inhaler as required, Terbutaline Inhaler 2 puffs twice daily long term, Isosorbide Mononitrate 20mgs Twice daily long term, Simvastatin 40mgs daily long term.
